I think that everybody expects like changing all visuals, maybe selecting all the stuff, equipment and so on.
To be able to create all from yourself to Chuck Norris.
Its not really neccessary.
There could be like standard visual customizations - body strength, hair style,...;
but all of other thing like equipment are bound to gameplay and should be looted during playing the game,
I would not mind at all to have just predefined visual for each role, if it would be "player's temper neutral.",
meaning that as youg boy in real world you won't be playing as some fat girl which kills everyone with rocket launcher.
Geralt is somewhat good example - old and not old, wise but stupid, strong but weak,... does not look like superhero in crowds on the streets.

Also would be cool if your cloths worn out as they take damage. Like in Batman Arkham (all of them?), as you progressed through the game, your suit would get ripped and torn and burned.

how is attractiveness going to be handled in this game? Is it going to be handled at all?

One way around it, that I can see, is specifying this skill in the game as some sort of personal charm, instead of physical appearance, that allows you to look attractive for other people, even when your character looks like it has been dragged face down through the asphalt with a fast moving car. In other words, when your character is ugly, with high attractiveness skill it becomes fascinatingly ugly. On the other hand, if your character is customized in a way to appear sexy, but it's attractiveness is low, it basically turns into Johnny Bravo.
