But what if Geralt accidentally hits the monsters with steel side first? Or are the silver and steel placed other way around on the other side?
 
that's a nice idea, but if we remember that the silver sword is actually coatted on silver, the silver part would be a little more heavy, and that is a problem, a big one for a swordsman...

It wouldn't be ineffective per se against a single person, you can deal a deadly wound easily, but the durability is an issue because silver is soft, so the chances of ruining an edge on the first strike are very high. So continued effective use is an issue. In the books Geralt uses silver sword only against magical beings like werewolves, vampires, spirits, wraiths, etc. For insectoids, draconids, ogroids, drowners, ghouls, etc. Geralt would use a regular steel sword. Games simplify this, steel for humans and humanoids (elves, dwarfs, halflings...) and regular beasts, silver for everything else.
 
The weight difference would be a huge issue. Also fusing the steel with silver would also be an issue, or if you simply coated one side with silver would lend a whole new issue in and of itself anyway.
