as far as I know there is no mod like this for the armour. The guys of Farewell of the White Wolf achieved to import some witcher 3 models, but it is not clear whether they are going to share their work with the community or not. The eyes are probably just a texture mod so you could try to achieve this by yourself, there are plenty of texture mods and - I think - even tutorials for how to make those mods on YouTube.

I have talked to the modders who are creating Farewell of the White Wolf and they told me that the kaer morhenn armour is not wearable and it's not possible to import it.
