It has healed, as much as that kind of scar can heal. In TW1 it was fairly fresh, if you remember it's origin in the Price of Neutrality DLC mission (it was CDPR's official, not fan-made).
Plus I think CDPR figured out that Geralt can't have all the girl fans in the franchise, he needs to have some competition
