I don't know if my problem is the same but it seems familiar. I'm playing on PS4 and witnessed the late spawning of NPCs regularly but usually I could fix it with waiting around a bit or meditating however now I've run into a Mainquest-Deadend. Spoilers for one of the late game quests following...
