Bring back insane mode - DM difficulty level with no saved game that can be loaded once Geralt dies.

I love this mechanism. I like the fact that sometimes I have to give up my current sword or other equipments because of the damage and I have no choice but to grab any available swords from the enemies that I had defeated. Help with the immersion. Just make sure you look after your silver sword.

Well, it actually makes a lot of sense. Level 20 bandits are more accurate with their shots so when they hit you, they hit you in your organs, which takes so much of your health. On the other hand lower level bandits probably only managed to scratch you with their arrows.
