Exact same issue happened to me. It glitched as a "failed" quest after I completed King's Gambit and Coronation. The green check marks are still there which confirms that you completed Cave of Dreams properly.

I don't think it was there until I noticed it after patch 1.05. I imported a TW2 save, but I know for a fact that I removed the neck tattoo in TW2.
