I know this might not be what you want to hear--but I did Windows Reset (keeping my personal files) and the game now performs as expected. Around 35-40 consistent FPS on my system on High settings, which is to be expected given my low-to-medicore hardware. Perfectly playable.
Dude; thanks for the suggestion! So I never made it past the character creation screen because, what's the point at 10 FPS. It was hard to even create a character. Started the game; 10-12 FPS throughout. About 10 minutes in and I am in the vehicle to Jackie's bar and the game suddenly snaps...

Splash screen starts off at 250+ FPS for the first few seconds while the CD Projekt Red logo appears, and during which the Windows mouse pointer is still visible (in the below clip you can see I move the pointer to show that it is smooth; FPS is still good)...
