Hello, Those issues look like data corruption. Not sure what PC platform are you on (GOG, Steam, Origin?), but you will need to verify/repair game files. Hope it helps.

You can start BaW without fear, issue occurs only for those who started Blood and Wine in 1.20 and left it when patching to 1.22. If you did not start BaW yet, you won't encounter any issues.

It is possible to break one's save file and make it act incorrectly in NG+ by using Exp Exploit or cheats. Did you use any? Normally New Game Plus is unlocked by reaching level 35 and completing the main story of W3 and leveling works correctly in this scenario.
