This happened to me and I did about 2 or 3 main story quests before I noticed the difficulty had reset. I should be done the story in a day or two and will report back if I got the achievement or not.

I hope this is the case as well. I just noticed this happened to me and I have done a couple of the main story quests already since 1.07t. Already have over 90+ hours into my Death March run.

Turn off Instant On in the power options until they fix the bug. I haven't had a problem since and played about 4 times. Also I think most reviews were done on PS4, where this problem doesn't seem as prevalent.

Happens to me after resuming using "Xbox Instant On". Put your Xbox into "Energy Saving Mode" until the bug is fixed. Glad I only lost about 45 mins before finding this out.
