Thanks, disabling depth of field in cutscenes and cinematic depth of field has seemed to fix the issue. I have 12GB of RAM and set my virtual memory to 12GB so I don't think that's the cause of any lag.

Ok I've found a strange workaround. Whenever the game starts getting slow I just load my last quicksave and the performance goes back to normal. Weird but I guess it's better than being stuck with my fps in the low 30's/high 20's
