I believe Phillippas Head is faulty anyhow. Find out whether another head works. In FCR2 Phillippas head is also missing.

I had exactly the same problem. Numerous attempts made. Then I have made before the fight a Savegame and have restarted the Game. Then on the last Limb I have hammered like mad the Mousebutton and the Spacebutton. This worked!

Do you have modded xml's in CookedPC/items ? Remove it.

Check in Game Launcher Menu under "Mods" whether all hooks are placed for the packages. Moreover, I advise to remove to you available (in existence) modded XML's in CookedPC/items.
