That's because you're using Full Launch Trailer appearance (http://www.nexusmods.com/witcher3/mods/297/?), by ThinOVintage, which also edits Geralt's face texture. You must set it with a lower priority than Lore Friendly Witchers.

While it looks, really, really good, (especially the scabbard) you seem to have omitted the "ring" on the middle of the grip ? (As seen here and there: I don't mind it being different though, it adds some variety to it.)

It really is interesting. Is it the only mod where this problem occurs ? I can't honestly think of what might cause the problem, especially if it's caused by Steam (considering the mod only modifies textures). Perhaps (though I doubt he has a solution) you could try to directly contact...

I just tried it, without any other mod installed, and it works just fine. Are you sure you haven't renamed the mod folder somehow ? (I can provide pics to prove it if you don't believe me)
