About the hard difficulty

So on the 2nd hardest level, "bone" and "blood" or something, is that true that health won't come back even out of the combat? What about the perk that allows faster return of health? Is that perk totally useless on the hard level?

Yes it's true. There is a perk in the "alchemy" tree which allows you to recover health during the day (and stamina during the night).
First one I learned.

So this perk works no matter what difficulty you choose?

It's not in the alchemy tree I think but utility.

I don't see why it wouldn't it would be quite ridiculous to put in a skill point invested perk that only worked on the first 2 difficulties haha

If we're talking about the same perk then yes it works (I was wondering the same before leveling it up).

Sounds great. I don't like super fast health recover after combat but it also becomes quite annoying to eat food after each combat.

It's the first skill in the General tree and yes it gently revitalises you after battle so you don't have to carry an orchard full of apples...

I'm playing on Hard and without any perks yet I regain health out of combat just watch your health bar it will SLOWLY regain bit by bit on it's own without you eating anything.

probably you have some gear-piece that allows it...

anyway that perk is mandatory for hard difficult...

If I remember correctly, first 2 difficulties you can recover health by meditating more than 1 hour or by eating. Hardest 2 (I am playing second hardest) only be eating. And don't forget you can eat during combat too! I don't mind this system, but for my play style I just end up spamming quen the entire fight and don't get to have fun with the other signs. I think I just need to man up and take some damage to deal more damage though.

This seemed hard and annoying at first, but I like it now and only died a handful of times in White Orchard. I did get the utility regen ability but more for the stamina instead of vitality. I found it pretty annoying in Witcher 2 to have to wait around for my health to slowly regen and then hit F5 for a save before the next fight. Food is a good alternative.

Playing on the hardest difficulty I regen 1 point per second without perks. So you do regen health in all difficulties, it's just REALLY slow. Check the character info page, the one with the whole page full of stats. Can't remember what button you had to press while in the first page but it tells you that in the bottom.
