I'm not experiencing any crashes related to long term play but delayed ambient sound when fast traveling to Novigrad, blurred textures on roads while running in Novigrad, npcs not loading properly, stuttering, objects and NPCs pop-in and the worst one: blurred screen every 10-20 seconds in Novigrad, Oxenfurt and dialogues. This game is getting worse with every patch.

I think I solved this by rebuilding ps4 database.

I wouldn't be so sure of that...I've been crashing at least once a day ever since I started NG+. Submitted a support ticket, now let's see how that goes...

Guys just rebuild your database on ps4, it may help.
My game did not crash for 1 week since I rebuild it.
