I am pretty sure that once you start New Game Pluss, Grand Master Witcher Gear becomes level scaled at your level at the point of crafting. So if you're level fifty when it's made, it is level fifty. So if you make a new set every 5 - 10 levels it will always be the strongest armour in the game. Also the Tesham Mutna Armour and Hen Gaidth Vampire armours are scaled. Infaft most of the best quest gear is in playthrough 2. However so far as I know Aerondight is the only item that keeps leveling up with you as you play. And yes once you add upgrade slots it is easily the best silver sword in game.
The only thing that really anoid me about witcher that I believe every piece should have three upgrade slots so as to make every piece enchatable. I would have settled for being able to add additional slots. But as it is a bit underwhelming.
