Are all loots hand placed, or generated somewhat randomly?

Are all loots hand placed, or generated somewhat randomly?

I know the diagrams for Witcher gears are hand placed, but I seem to notice some repeated loot locations between my two playthroughs, which makes me wonder if all loots are hand placed in their specific locations.
