Berengar's Blade Quest Bug?

I have been trying to get the quest to activate on both of my playthroughs and even after I pick up the letter in the chest at Kaer Mohren, I never get the dialogue option with Vesimir to start the quest. I have tried it before starting any quests in the area, before finishing ugly baby, after it and also once the game was completed.

Am I doing something wrong or is this quest just bugged for PS4? All platforms? Trying to go for a platinum trophy and 100% on my 2nd playthrough and this is holding me back from progressing...

Has anyone been able to get this quest to work or complete it? If so can you tell me how I can get it to work?
 
I was JUST about to make a thread. In my case... the quest progressed just fine. Went to the cave and battled the chort. Now, time to pickup the diagram...I see a red sack with my witcher senses (assuming its for the quest then) but theres JUST ORENS and the sack dissapears...the quest cant progress now. Ugh...
 
Same problem with Berengar's Blade. Tried reloading several different saves (right after arriving at Kaer Morhen, after finishing the different subquests of Ugly Baby, after completing Ugly Baby) but nothing worked.

Same here except for me the quest wouldn't even start. I just went to the cave to maybe trigger it by finding the sack, but there were only orens inside and it vanished. I'm losing count of how many bugged quests I've encountered.
 
I'm on PC, same bug. I got Berengar's notes from the glowing, red chest in Kaer Morhen, but no option to talk to Vesemir about it.
 
Hi all, i meet same problem, quest for me activates after i found book, dont remember how it named, i found it in chest under big spiral staircase in big tower near kitchen, where room of Yen on the top.

Chest is to the right of the head.

Bit late to the party on this one.

But just for anyone that is interested.

I am on my second playthrough.
Just as with my first playthrough, I cannot initiate this quest.
I have the notes, but cannot talk to Vesimir about it.

This bugged the crap outta me because of plot reasons.

So this time around I did some digging.

Ultimately, the reward for the quest is a sword diagram.
However, it seems this diagram is not unique, and you may have already found it.

My best guess is that if you already have the diagram from elsewhere, the quest is already considered concluded and thus will not start.
Pretty sure you can still go through the motions, but the quest will never activate, there will be no dialogue, and there is no reward (because you already have it).

Hope that helps anyone that is confused by this quest.
It sure as hell bugged the crap outta me.
 
So THERE IS a quest behind this note. I always had my suspicions...

And yeah, you guessed it, it didn't activate in my game, either.

May be this bug occur if we have already Tor'haerne schematic ...
On my side, when i enabled this quest, i had already found this schematic somewhere...
 
I thought finding that note was just a bit of "fluff", you know, something that helps us with immersion, I had no idea it was meant to trigger a quest :(
Mainly because it reminded me of the note you find in the garden at the palace at vizema.
 
According to the response I received from support, this isn't actually a bug. If the player already knows the recipe for Tor'haeme, this quest becomes unobtainable/unfinishable. That's just how it is according to Kuba Woźniakowski.

Great... so does anyone know a way of removing learnt schematics with console command? Unfortunately, even "cleardevelop" doesn't reset them. This is just poor design, this schematic should be unique or the quest should start even if you have the schematic. Currently it seems like a punishment for exploring.
