I guess it's not just me then, at least I know it's definitely a bug then and not something I screwed up somehow. Bloody annoying though, means I had to go back to an earlier save to avoid it, removing my Kings Gambit progress. Hoping this is fixed soon, I don't wanna continue the game until I know quests won't be randomly failing for no reason and screwing up my progress any further.
On Xbox One, and also ran into this problem. Finished out the quest with all "green" quest components, in the journal, but it is listed under "Failed" missions.

This has happened to me as well. I reloaded and am waiting for fix before continuing game... so need it asap

i had a few quests where at a certain point you had to make a decision. that decision a certain way could fail one quest but could start or further a different quest. i'm still not sure this isn't one of those situations where something earlier in the game, something during, or something after the quest determined it forfeit.
I also had the same thing happen. Quest completed, then after "King's Gambit" it was suddenly in the "Failed" section, although all quest objectives are marked as completed.
While this will be obviously patched for new games (at some point), I'd like to know how CDPR fixes this bug for old, existing save files? I think it would be about time to add some kind of "complete quest X" command to the debug console and tell us what it is. I am personally still stuck at the "Get Junior" quest because I can't talk to Vernon Roche in the partisan camp. Can't progress the story at all.

I think you must do the cave of dreams quest 1st then the light house quest next do it that way and i think lugos jr cant die in fact he will help at the kear morhen fight i heard about this on another forum hopes this helps you guys. Just did it this way still bugged out oh well got to wait for them to patch it.

I Just noticed that even though I had completed the quest, it's now gone into the failed tab, and the only objective present on the failed tab section is that i didnt join Lugos outside the cave, despite finishing the quest. This is a little annoying
Same problem happend to me! I finished the game and this quest is the only one I have marked as "unfinished". I'm playing on PC. When I click on this quest i see green marks that I've done everything right but the quest itself is "unfinished". I hope they will fix it in 1.07 (with Zoltan missing card under the Hang Man's Tree, Svana NPC missing so I can't finish final horse race).
