Bug on "Hatred Symbolized"

As in the title I have a bug with the quest "Hatred Symbolized".
In the act 1 I chosed the Vernon side and so I'm in Vergen to find the banner, the spear and the sword.
When I go in the caves to find Saskia and Zoltan, tough, Saskia says "Here's the sword" and disappear.
But I have no Commander's Sword in inventory and the quest remains stuck on "Find Zoltan in the caves".
I tried a mod wich adds any item you want in any merchant you want and bought the sword.
This complete the mission "Hatred Symbolized" but in the mission "Blood Curse" i still have "Find the three objects (2/3)".

If you have the sword, the other remaining objects are the Dun banner and the spear head. Saskia should tell you that Iorveth lost the spear head to Skalen. after that you should be able to talk to him and won the spear head through playing dice.
if that didn't help, you might also try to load a previous save before crossing the mist.

I have the spear head and the banner too but the "Blood Curse" mission still states 2/3
Later I'll try to load a previous save.

I tried to reload an old save but nothing changes...
Any other ideas?

If you're playing the Steam version then checking game cache might help solve issues like this. but if you're playing the GoG version, sorry I have no idea.

It's the gog version...
