Calling Roach
When you call on Roach, he's sometimes stuck behind a fence or similar structures. I one time saw him coming from under a bridge near Novigrad, but got stuck in the road (halfway) - you could only see the upper part of the horse. Perhaps some algorithm tweaks may be needed in this department.
