Happened to me as well.
First, check whether there is a siren stuck on the 2nd floor that cause you to enter combat mode.
Second, in case the siren is dead already, fast travel away from the lighthouse and then come back. The ladder should work properly now, or at least this worked for me.
Meditating on the spot did nothing in my case, but you may try it as well.
