Constant combat freezes

Hi guys, I have been playing TW3 (1.31A) for about two days now and I kept having combat issues such as the player freezing and not being able to move. Sometimes, my player would also draw swords but not able to attack. Anyone has any idea why this is happening?

0.) Using any mods, or have you every used any in the past? (If so, remove them.)

1.) Delete everything in your ...Documents\The Witcher 3... directory (EXCEPT the gamesaves folder -- keep that!) Then, verify your game through Steam / GOG / Origin.

2.) Set the following:

GPU Control Panel
Vsync = Controlled by the 3D application
Frames to render ahead = 1
Monitor Refresh Rate = 60 Hz

In-Game
Fullscreen
Vsync = On
Frame Limit = 60

Let us know how that goes!
