My game doesn't start since day 1. I alwaysh have this "Whoa! Cyberpunk 2077 has flatlined" error.
I tried AVX fix (my cpu supports avx btw), Reinstalled 3 times, updated gpu driver and etc. I have tried all possible solutions you have described here but nothing is changed. Still same problem when I click play button on launcher, or trying to launch with exe file. I can't understand what is wrong.
Here's my system, pleae help me this game turned out to be a huge disaster for me.

Intel(R) Core(TM) i5-6600 cpu

So its gotten better but long missions where I can not save make me nervous as if its going to appear somewhere
its usually at the end of those long missions

OK, I've found out my problem's root cause, therefore I created an account to post here. I think many people would benefit from this, so I kindly ask the moderators of the forum to highlight this, and developers to find out the bug :

To anyone who has the following issue, CyberPunk crashes during start with error "Cyberpunk2077 has flatlined". When you click on More details, The details given is as below :

Message: Gpu Crash for unknown reasons! Callstack here is probably irrelevant. Check if Breadcrumbs or Aftermath logged anything useful.
File: e:\r6.patch0hotfix2\dev\src\common\gpuapi\src\dx12\gpuapidx12error.cpp(40)

The root cause of the problem is the DirectX12 C++ API library. It crashes on Windows 10 version 1607 (LTSB or Standard). I've seen some other root causes but this was my problem. Upgrading to a higher version of Windows 10 fixes the issue.

I hope this helps fellow gamers.

What is your windows version? If it is Windows 10 version 1607, then that is the root cause.
I've put a post here about this issue, I managed to tackle it.
To make sure you have the same root cause, run the game and in the error message box, click on "view details". If you see an error like :

Message: Gpu Crash for unknown reasons! Callstack here is probably irrelevant. Check if Breadcrumbs or Aftermath logged anything useful.
File: e:\r6.patch0hotfix2\dev\src\common\gpuapi\src\dx12\gpuapidx12error.cpp(40)

This might be due to your windows version. The problem is in API library of DirectX 12. Upgrading your windows to a higher version might fix it (not fixes or patches, a higher version, for example I had 1607 and upgraded it to 2004 and it got fixed)

Also, someone posted that Raytracing might cause this as well. My GPU is 1070 so I have no raytracing.

Given the recent announcement, it doesn't sound like we will be getting another patch till January or February....

What gives?? Seriously angry right now. Day dot I could play fine, few GPU crashing issues but still got a good 5-6 hrs gameplay. THEN came the WORST 'HOT FIX" in history. Ever since then, as soon as I hit play on the CP launcher its instant crash (flatline) with an unknown error. I meet all the minimum specs. NO IDEA whats going on.

Ive upgraded from win7 to win10 incase that was the issue - it wasnt. All drivers are up to date, BIOS, etc all current.
I REFUSE to pull apart my PC to get this POS game working. Im beyond cranky. And when I hit the support button I CANT EVEN FIND a place to ask for OFFICIAL support from someone at CDPR, or open a ticket, etc.

SO many years waiting for this game and THIS is the experience we're greeted with. I'm BEYOND annoyed. This ISNT acceptable CDPR [...]. And their apology is mainly directed at console and STILL only updates are jan/feb?? Meanwhile,a lot cant even play??

We'll be getting a patch in a couple of days, the jan/feb patches are mainly for last gen consoles.

There is no chance that they will leave a PC version in this state. I'll bet that CDPR will improve stability in next week or so.
They have to, otherwise, they won't be able to sell anything ever again.

And for the consoles, well you are screeeeeewed.

Played for 3 hours two days ago without any crash. Since yesterday the game is crashing after 10-20 minutes. I completed Act 1 when I played 3 hours streight, now I'm in Act 2 - maybe this can be a cause for the crash?

My system:
AMD Ryzen 7 3800X - tried with and without OC
ASUS PRIME X570-P - latest BIOS
Nvidia GTX 1070 - latest driver and firmware
Crucial 2x8GB DDR4-3600 - tried with XMP/DOCP and without, tried at 3600, 3400 and 3200.
Crucial P1 1TB M.2 SSD

The crash appears suddenly, the game freezes for a few seconds and then CTD.

I tried the Cyber Engine Tweaks mod, tried low settings. I also played in windowed mode and 1080p (usually I play in 1440p).
Running the game as admin and set the user config as read only also didn't help.
Did I miss something?

To anyone still running Windows 7, I know your pain, I loved that OS as well and dislike Windows10...But due to my constant crashing and poor performance I tried every possible fix in here.
Today I said 'fuck it' and had a clean Win10 install.... guess what.

Crashes reduced (only played a few hours, could still crash), stability increased by a lot(less random stutter), and the FPS is a LOT smoother while the image looks crystal clear now on the same graphics options compared to before.
I didn't believe that before, because a lot of people here had similar crashes (random startup flatlines + gpu crashes mid play and on mirrors, my options menu was bugged out) while reportedly on a fully up to date Win10 install. But for me it really is like night and day right now. It's not perfect, but looks and starts like a decent game now.
I suggest to you to do a clean Win10 install as well, if you can. Even if it is just on another drive or partition than your Win7, it's worth a try. If you have a valid license you get a free upgrade still.

You can't, I repeat CAN'T play on Windows 7 well.

I trusted CDPR's requirement sheet but it's kind of a lie. I suggest to them to change that immediately since it really kind of is false advertising and creates a whole lot of problems.
They may improve stability over the next few months but we bought the game LAST WEEK. We wanted to play LAST WEEK. Release the game in February if it is ready in February, not in December, getting our hopes up and destroying your own rep. You're betraying everything you stood for with this EA BS...
I know the leaders of CDPR 'took responsibility' and 'apologized'.... but maybe they should instead take a good look in a mirror....and immediately crash upon doing so, being unable to play their own game. LMAO... I'm very sorry for your employees who must be crushed by the bad feedback and would like to 'apologize' for my honest tone.

Angry customer rant over. Have a nice day now, seriously. I'll go blast some scav gonks, finally.

I haven't been able to get past this, I've tried everything except upgrading to windows 10, too many other things make it not worth upgrading, hardware and lack of drivers ect...

"For boxed versions, please first try to get a refund at the store where you bought the game. Should this not be possible, please contact us at helpmerefund@cdprojektred.com and we will do our best to help you. Starting from today, you can contact us for a week up until December 21st, 2020. "

Every time I launch I get a flatline, It's nice that they are offering refunds for the 5 days , but ask that you hold faith till January when they can fix things, oxymoron of we failed your trust, but trust us more by not asking for the refund by the allotted deadline because were so going to fix it ..eventually, despite never even confirming support emails, and asking you to jump though a lot of hoops that amount to no progress. Don't worry that it will sometimes take longer than 5 days to attempt a refund from where you bought it... a 5 days window is enough to say .. well we had this opportunity for you .. we did accommodate people with lip service...

Well I have read through all of this forum and would just like to give my two cents for my flatline error....
Within the dense parts of Cyberpunk city in a car or bike turning in 3rd person fast would hinder my frames down to about 50 to 40 fps... regardless of the graphical settings high or low or ray tracing etc... I am running on I7900K CPU AND MSI 2080 SUPER TRI... ram speed which I will get to in a sec runs on default MHZ of 2133..but these corsair are tested at 3200mhz ...

So I enable my XMP profile and there we go running at 3200 ... timing and volts as per ram specs can run games like OW for hours but besides that as soon as I enable this oc on the ram....my lag and fps in game stable at 60 to 70 fps even in these crowded dense areas as well as driving and turning the cam feels smooth.... I felt bliss. It completely fixed my fps drop in city's and in car....

But then it started crashing.... randomly between 10 min in to one hour or not even starting....Only with XMP enabled.
Without XMP no crashing regardless of playtime Fps outside of major city areas around 80 to 90 but sluggish at home area jumping between 40-50-60.

I do not know if this helps anyone, I did read a post here about someone suggesting disable XMP. I do have the newest bios for my Apex xi motherboard. I do have the newest win version. Only crash I have so far is because of ram via XMP.

I played with and without DOCP (Asus XMP name) on my 3800X with DDR4-3600 RAM. Still crashed every 10-20 minutes.

I discovered that all works well including Ray Tracing options activated. There is, however one problem. On my SLI configuration CP2077 tries to run at 60fps but it crashes on interface screens as before. I tried to define FPS LIMIT at 30 and then I played without problems for some 30 min. A word of explanation: CP2077 does not support SLI but it runs faster when SLI is enabled. Some other games do the same.

Unfortunately, when I tried to sell a larger number of inventory items, it crashed while on the selling screen, as before.
