I found the following audio bugs in game:
1. When Jackie dies inside the Delamain car, I can still hear Jackie breathing which is ruining immersion (this audio bug does not always happen).
2. In the mission where Panam and V overcharge the Satwave Power Plant, the loud static sound does not stop, and continues to play during the AV chase (requires a save and reload to fix - this audio bug also does not always happen).
3. In the Peralez quest "Dream On", the laptop on the table (inside Jeff's office) contains the file of his campaign ad video. This ad unfortunately does not have any audio playing. It would be cool to have the audio present there as well, like when that ad plays on the TV.
4. Stealth takedowns on major bosses in game are lacking audio, watch this video for the specifics:
5. In the quest "Big in Japan", at the very end when you select the dialogue option "My payment", Dennis replies with "I know. Here... may this blade serve you well" the part "may this blade serve you well" sounds completely different to Dennis's voice.
6. An audio bug that has been present throughout multiple patches is the sound that "indoor beaded curtains" make when you walk through them. This sound effect can sometimes bug out causing it to keep on playing in the background without ever stopping. You need to restart the game completely for this audio bug to go away. I have attached a picture below to anyone who is unsure of what I mean by the "indoor beaded curtain" (this audio bug does not always happen).
Non-audio bugs I found:
7. Stealthily falling/jumping on top of an unsuspecting enemy results in a finisher move, but this finisher move looks weird if you have a Sandevistan active (it looks very rough and weird looking).
8. When Jackie and V goes to get the "Flathead" bot from Royce, Dum Dum controls the bot, during this scene his red optic turns blue, but there seems to be some sort of a graphical glitch in his optic that needs fixing.
9. The "GladOS" Delamain car still remains parked in Pacifica (where she sets up the ambush) even after completing the Delamain questline for some reason.
10. Brick's niche still appears in the Columbarium even if you saved him in the quest "The Pickup".
11. In the quest "Violence", as V enters the No-Tell Motel room, the very first subtitle is "Lizzy Wizzy: Started to think you wouldn't show" spoils the mysterious woman's name as Lizzy Wizzy, while her next line is delivered as "Mysterious Woman: Why not?".
12. In the quest "Riders on the Storm", Panam says "Oh yeah... We even have a radio in here. Take it for a spin if you like" but the Radio is disabled in the van so we can't change the radio station at all.
13. In the quest "Riders on the Storm", right before we arrive at the Raffen Shiv camp to save Saul, the visible Sandstorm above the Raffen Shiv camp seems to have some sort of a texture glitch where it flickers when you look at it or slowly look away.
14. In the quest "The Hunt", Randy's Antique Key (which is used to unlock the locked cabinet) keeps on spawning even after being picked up by V. This bug also has been present throughout multiple patches and is still not fixed.
15. After completing the quest "The Prophet's Song", there is a bug where if you come back later on and try to "continue conversation" with Garry's friend named "Bum", she despawns.
16. In the quest "Play It Safe" (the parade quest) there are some NPCs that are floating in air. I think these NPCs were on the Dashi Parade Floats but somehow got "desynced" staying on the floats as they move, or may have been spawned there incorrectly? I don't know, but the below image should be helpful. I have seen this bug many times across all the patches and it still has not been fixed.
Quality of Life (QoL) improvement suggestions:
1. Also, why aren't we allowed to properly order the quickhacks? It is very annoying that once you equip a new quickhack in your cyberdeck that it shifts other quickhacks in the hacking UI so that the most recently equipped quickhack appears at the top. Instead, it would be great to have it so that the order of the equipped quickhacks in your Cyberdeck slots is the order shown in the hacking UI.
2. Another improvement that would be awesome to have is to be able to walk freely inside the NCART train. Just make it so that V can't use weapons or quick hacks so the player can't disturb the peace inside the train.
3. Be able to permanently set a default CrystalCoat color in the vehicles we own.
