On Iorveth's path, you can get into Philippa's house late in Act III:
She leads you and Iorveth there after you rescue her. You can't get in before then.

On Roche's path, you can get in there somewhat earlier:
After you talk to Radovid.

There's no other dragon scale to be found.

The Operator may be accessed the same way on either path, but only if you grabbed Malget's notes earlier. Nothing on Iorveth's path guides you to the place you find them, so this is easy to miss out on.

Supporting completionism was not high on the developers' objectives with this game. It is frustrating if you want to complete everything possible. This is intentional, not a bug.
