Ok, I was getting the same thing and it's possible that the Prefer Maximum Performance thing the OP suggested has worked. I played for well over 3 hours without a problem.
The reason I say probably is because I recently overclocked my cpu (and also bought a new graphics card) just before getting GTA V but never properly tested it. GTA V was crashing too, randomly, sometimes after 5mins, sometimes after an hour, same way TW ended up doing. I assumed it was just the game since other people had the same thing but I was also worried that maybe the g-card was defective. I never got around to any testing and forgot about the overclock til yesterday.
However, when Witcher started crashing too, in the same way, I decided to increase voltage and also use the OP's suggestion. I was too impatient to try one at a time so I did both before I booted Witcher up and, like I said, it seems to be going fine.
TLDR I thank the OP for their suggestion since it may well have fixed my game (possibly GTA V too, gonna check it now) so I recommend everyone try it.
PS: For other overclockers out there, this might be interesting. Talks about how Prime95 and other programs don't necessarily reflect what will happen when you max cpu AND gpu at the same time while gaming.
