[General] What is actually required to get a journal entry of a character?

Hello, I've just started at chapter IV, talked to some important people (lady of the lake, the hermit) and I noticed that those two didn't show up in my journal, so I'm wondering if this is a bug or if I have to do some quests for them or something else, since talking to them with every dialogue option at that time didn't work.So what is actually required when getting a journal entry of a character?

I've looked at the wikia, and I think both of them are supposed to end up in the journal, at least the lady of the lake is.I've started a quest with the hermit, though he isn't showing up.

Whisperwind is right, no every NPC you meet in the game or who gives you a quest gives a character entry in the journal. The Witcher wiki has entries about all relevant NPCs in the game and that's great 8)

True, but as I said, I should get an entry from at least the lady of the lake, and the first entry according to the witcher wikia, says:
On an island in the middle of the lake I encountered a supernatural being known as the Lady of the Lake. She is a nymph or a vila. Both the inhabitants of the nearby villages and the monsters of the lake worship her.

I just find it weird that I haven't gotten an entry yet.

I'm not sure about this, but maybe you get the entry from reading a specific book telling about the object in question or talking to a specific NPC who will give you details of the object in question. Either way, by attaining the sexcard of this particular person, The Lady of the Lake, you are bound to have the journal entry. Otherwise, get involved in conversations like with the Resolute Girl or Townsfolk and read as many books as possible. Something will turn up. :peace:
