One thing I've noticed is that we're not often given the choice about how we want to go about a mission. Ghost Town is a good example, or at least the one I'm currently on and noticed this pop up. We setup an ambush around the abandoned city, I pull the power and Panam tells me to go do something, but I'm just thinking "sword swinging", and immediately my V says "cover blown", Panam freaks out and the fight ensues. I would have rather this situation had some options, and other quests as well, with maybe Panam asking me how I want to approach this and give me several choices, like stealth (default), guns blazing, up close and personal, etc etc, so she knows what to expect from me. Some food for thought for the future.
