0.) Are you using any mods, or have you ever used any in the past?

1.) Try running a verify / repair through GOG / Steam / Origin.

2.) Ensure the game is not installed anywhere under Program Files or Program Files(x86). Protected directories can cause odd issues with games. Install to the root of your drive or a folder of your own creation. For example:
C:\The Witcher 3\...
or
C:\MyRPGgames\The Witcher 3\...

3.) In Windows 10, go to the ...\Documents\The Witcher 3\... folder and make sure its Permissions are set to "Everyone".

Okay. Nuts.

We'll need much more specific info on the issue to begin troubleshooting, then. Please, answer each of these individually:

1.) Where did you buy the game (GOG / Steam / Origin), and what version (Standalone TW3 + HoS + B&W + 16 Free DLC, or the GOTY Edition)?

2.) Can you confirm that a.) the base game, b.) the two expansions, and c.) all 16 DLC are installed?

3.) Explain your installation process. Did you manually install anything after the initial installation was complete?

4.) When did the problem first appear? Can you remember when and where you were in the game when it happened?

Most likely, there are files that are corrupt / missing / not correctly installed. If that's not the case, then it likely boils down to a very odd issue with your system config (drivers, Windows Security, etc.) That'll be much more fun to sort out. ;)
 
It would be interesting to know more about your computer. CPU, GPU, RAM...

Reinstalling GPU drivers or rolling back to an older graphic card driver version can also be worth trying.

Experimenting with the hair related menu settings is also worth a shot...
