Have you died in the game because of a bug or frozen screen while the game still continues?

And if so, what happened and where?

I once climbed up on a boat, then as I did - I flew up the sky and fell down to my death.. on the boat.
I once fought ghouls and my screen got frozen while the sound & game still continued (I could hear my death).

I once was fighting some bandits, but when it came down to only one left, suddenly it glitched and treated him like a normal non hostile NPC, couldn't cast signs at him, and couldn't attack him either. But he could attack me just fine.

Haven't encountered death due to bug/glitches yet, all of my deaths are due to my own mistakes(wanted to jump into water but landed on land instead from very high ground) and death from being killed by monsters/bosses :)

Not me, but I often died because I got stuck between objects in the world and cannot jump in combat. Rolling/dodging do me no good as well coz surrounded by objects.

Just recently during mission with Triss the screen froze in fight for 5-10 seconds, the sounds were ok. Then it unfroze and the gameplay continued from where it stuck.

Something similar happened to me, except only the camera flew up and slowly sank down behind Geralt again. And it happened as I was getting out of the boat.

No it never happened to me. Maybe there's some background programs running that is causing it for ya.

I am always lucky with those things. I almost never encounter bugs or glitch and even if i encounter them, they are mostly harmless or so minor that i rather laugh about them.

Once i jumped and rode down a mountain with my horse and it was literally running with its tail, all 4 legs pointing forwards. Just hilarious.
