Great suggestions. This worked. I verified file integrity on steam. IT fixed everything.
Ironically I never had any T glitches for characters or heads popping out of car while driving, and now I have all that. But at least I can play.

PS4 Pro user here. In my case main issues:
- Wearing any kind of hat causes V to always appear bald in mirrors
- Frequent bug means I cannot loot items - the loot icon is there, but no matter how hard I try I cannot get a "get" option. Especially when bodies/drones fell next to objects.
- It crashes A LOT. Even after the last patch, averaging every 2 hours or so, most commonly when entering a new area that has to load.
Otherwise, an awesome game. Just wish there was a way for us to recolour clothing and/or change V's minor details (hairstyle, colour...) in mirrors, or to wear hats (for protection) but not "show" them (like Mass Effect and other games do with the helmets).

Another bug I have noticed repeatedly is that upon saving game > dying > reloading save > my inventory of weapons changes to what it was.
So like, I have a pistol > machine gun > shotgun as my three choices, when I die the pistol stays the machine gun disappears from the menu and goes back to inventory and the shotgun is still there, so I was doing something where I was dying a lot and each time I died I needed to go into inventory and reset the machine gun back so I can use it each time.... after a while this gets very annoying.

Aside from this, very often, after reloading quicksave or autosave makes the screen in game have black bars on left and right.
Good thing is that hitting "Esc" and then resuming the game fixes this...

I also get this and another thing after reloading, if you save while you have a surveillance camera turned off, upon reloading, that camera will be active again.
Is this what is causing the tree to render before everything else and overlaying IN FRONT OF EVERYTHING?

The reason for the bugs is, cd project were forced because of timepressure to outsorce some stuff to a studio in montreal, kanada.
This studio also made floating questicon in middle of the screen and TAA unable to disable. By accident they also deleted the female V 2018 appearance and dont inform cd project before realease about it, now it would take months or years to restore it.

After updating to this patch in Xbox one I experiencing massive slow downs in middle of the action, ruined my experience.
SAME HERE cant patch because have only 50 GB free

I suddenly found this. That Johnny Silverhand left hand stucked or glitch on my left arm. its look weird. I try to die, reload save, fast traveling, reopen the game, and etc, but it still like that. its part of the story ? so could be like that ? or that was a real bug/glitch.
Anyone got glitch like me ? or anyone know how to fix this ?. its really look really weird on my eyes

When I am cleaning gang hideouts, it says violence/force permitted by the cops or something like that, then during the fight on gang premises it says you are doing ilegal activities and police will investigate, and then cops show up and they start attacking me...

Not yet. I tried one more time after few quests, but still not working. I finished atleast two other quest where I need to use the computer the same way and they worked. Now, this one stays. As one of the last quest in the area.
I still get crashes occasionally when i get to a city area with a lot of fast travel machines, do a crime side mission then go to fast travel and i crash
