If you want to stay out of the way of enemies that can one-hit you, you have to roll. If you want to do so without using Quen as insurance, you will have to get good at timing your rolls.
The Kayran fight is probably the worst part of the game, VERY unforgiving even on normal and then horrid QTE's in a boss fight is disgusting gameplay (QTE's in ANY fight is horrible infact).

I agree. It must have taken me at least 5 - 7 tries, whereas I breezed through every other boss battle in the game in just one try (including the -second last- boss....).
OP, you need to 'Quen up', as you say. Then Yarden the tentacles, standing at the trap spot as the tentacle land. With Quen, you'll be able to withstand the first blow.
The dragon tail is pretty easy to evade, when the dragon stops crawling along the side of the tower, roll to the other side of the tower. It can't hit you when you are as far away from it as possible. The safe-zone is small, but it is there.
You can disable SOME QTEs during boss fights in the options menu but it still leaves the horrid button smashing QTE's. Its the "Difficult QTEs" option.
Fact is there shouldnt be ANY QTE's in fights. Takes away the sense of accomplishment you feel when you beat the fight on your own rather than have an abvious game mechanic do it for you.

Yes, I agree. It would be much better with a button "catch the tentacle", another button "jump on the dragon" and another button called "destroy the bridge".
QTE are the best way to integrate player's action into a cinematic. The real fight with the dragon or the Karyan are before the QTE, or between the QTE. The QTE happens at times where it would have just been a cinematic without QTE. That is not a perfect solution, but cinematic are there to make some fights epic, and the only way to keep cinematic while still integrating player's actions are QTE.
Two different school, take 2 quite very good modern times CRPG.
Mass Effect 2
The Witcher 2
In Mass Effect 2,
When you go through the relay, you have a cinematic which triggers. Your ship is fighting, you see your crew activating, but you're technically in a movie. You do nothing nor can help anyhow. The cinematic is great, but that's a cinematic
In the Witcher 2,
you fight the Karyan, you kill several tentacles, then you have to catch one tentacle. To catch the tentacle, you have to press this button, then smash your mouse button. Without those QTE, it would just be a cinematic.
So in the end, QTE are just human made action into a cinematic. It's still a cinematic, nothing more. Should we remove the QTE because it doesn't add much to the gameplay ? why does it really prevent anything ? it definitely didn't prevent me to enjoy TW2 scene compared to ME2 scene.

This is what Easy and no QTE's should do throughout the game--IT DOESN'T. Well, the opition isn't no QTE's...it only turns off 'difficult' QTE's--there's the rub.

The bottom line for me is _always_ how many times I have to replay something with no clear understanding of what I need to do to get through it. I'm actually pretty good when it comes to the QTE's but whacking away, timing hits and dodging and surviving on Easy even has been a pain in the ass.
And when my ass starts hurting to much and I've retried something a half-dozen times I turn on 'unlimited vitality' and 'unlimited vigor' so I can hopefully get back on track with the damn story. I would say dodging and blocking are my weakest skills in the game--I tend to ignore using both of them. I'll try to work on that.
The Game Guide has been very helpful suggesting what to do in certain battles.

I don't know - I have a hate/love relationship with QTEs, but, if I had to choose, I would say that QTEs are an added element that is worth keeping. For instance, in the Kairan fight, the QTE actually makes perfect sense, because you're not technically fighting when the QTE activated, but rather stabbing (I know that's still fighting, but I mean it's not a regular fight, it's just a last second...ehm...quick time fight), and the QTE really gave it that feeling of urgency, almost panic. I am not totally sure why people are so anti-QTE, it's sort of fun, when done right, and I think this is where the hate issue comes into play. The QTE in this game isn't very clear. Something just feels wrong with the way it's done. Maybe the keys flashing are too far apart, or are too small.
The thing with the Kayran is one blow from it's tentacle reduces your Quen to zero. Also at that point you don't have the Vigor to keep casting it, and even a couple of blows can be fatal.
I always used an Invulnerability Cheat(500-second Quen) only for this battle because I hated having to restart the fightt every time I failed to trap a tentacle or dodge it in time.
