How to dodge swipes?

If you can get even the first Vigor +1 before this fight, it makes it a lot easier. Otherwise, you have to get good at dodging.

Wow ratsneve, complaing about the Kayran but you won't use Quen? Sad. And rolling is great, simply purchase the Footwork skill, available right off the bat when you get the Sword tree. Very simple strategy, just drop and yrden like Sile says, then roll. Of course, always have Quen on as a precaution - realistically, Geralt would. He doesn't have a deathwish.

Never try to rush fights with severe damage penalty. You have enough time to think things out on both Kayran and Dragon fight. Also, you don't really have to rely exclusively on rolling for Kayran and the Dragon fight. If you notice the Kayran raising its tentacle, a quick strafe is enough to save you depending on how close/far you are from the core of the boss.

For the Dragon fight, don't get greedy. Land 1~2 strong attack then roll out to the center, run back and repeat.
