When i not forget first you must finish quest from Arinbjorn (quest to remove curse from Lighthouse) and before King Gambit Complete (when complete Cave of Dream will never show)
after finish quest and want to collect reward there is some accident
it will lead you to Madman Lugos, and they will pay for your crime on Arinbjorn --> for payment they need you to help their son (Cave of Dream Quest)

Also, a word of warning. I had a surprise on my last playthrough.
So if you happen to wander into the pub in Arinbjorn, make sure you speak to Jorund.

I walked into the inn at Arinbjorn, and it triggered the starting scene for the Lighthouse quest (The Phantom of Eldberg) where the two hostile guys approach you. As I was only there to play Gwent, I *didn't* speak to Jorund after the exchange. When I went back later and did the rest of the quest, Stranger in a Strange Land didn't trigger. (And no, I hadn't already done King's Gambit).
So if you happen to wander into the pub in Arinbjorn, make sure you speak to Jorund.
