I give up, anyone know how to cast alternative signs with KB/M?

I've tried everything, and I am only able to cast alternative AXII about 30% of the time if I successfully parry the foe first. (and yes, it is equipped in my active slots)

I've tried holding down the Q button, tried holding down the shift button while casting the AXII, tapped on the Q, held Q while pressing every other button, etc.

Help please?

Alternative Axii is tricky because it requires both a continuous pressing until the effect becomes active and anyway it is chance based. So you must be sure to not be interrupted while sustaining the effect long enough for it to fire. Alternative Axii is similar to how Axii worked in TW2, actually, however in W3 is more difficult to pull it out given how you are many times surrounded.

You need a progammable mouse/keyboard that can record multiple keystrokes aka macros. Hardware that utilizes Razer Synapse or Logitech gaming software....

Oh no, I'm not talking about being interrupted, I just can't cast the sign for the most part in the first place even if I am facing one enemy at a time.
The continuous pressing doesn't work though, each time I click "Q" Geralt just casts the regular AXII.

Did you move that ability over (drag and drop) to the skill tree?

I do have a Razer keyboard.
wow! Okay, so I just did a Macro bind to Q and it works now! although I have now traded regular AXII for alternative AXII.

Yup, I mentioned that in the OP.

hmm strange. Works perfectly for me.

Single tap = Stun
Press and hold, even just for a second = Mind control
