Cool in this game is the ability to stay calm under any circumstance, not "looking cool".

According to the rulebook, Cool measures how well characters stand up to stress, pressure, fear, physical pain and/or torture. It's also a measure of how "together" your character is, and how tough they appear to others. Finally, it will also affect your Street Cred.

You have this in Fallout 4 with each bodypart affected if hit by bullet(s) or melee-weapons. You even get concussion if hit on the head with blurry vision for a short time.
