Increasing Geralt's Toxicity Tolerance (Solved)

I seem to recall from my first playthrough that Geralt was able to consume 3 decoctions plus maybe another potion without suffering damage from having too high a toxicity. Now one decoction fills 70% of the meter, in my NG+ game. I must have forgotten something, as acquired tolerance is maxed and equipped.

Update: Problem solved, it's a bug. I re-equipped said skill and that fixed it. Geralt's toxicity is now at 265.

You need the alchemy skill Acquired Tolerance to raise the toxicity cap. You also need the alchemy recipes to get the bonuses from it. Heightened Tolerance often gets a slot too since it raises the threshold. Also, make sure you equipped/assigned the skills.
