Minor Glossary Glitch
Spoilers below, so don't keep reading if you haven't gotten past Kaer Morhen....

On the quest with Yennefer to save Rita from the prison, after doing so you get two glossary updates. One for Rita and one for Sile. The problem is the Sile de Tanserville one mentions that Geralt felt sorry for her after seeing the torture she'd endured and let her die with dignity. The problem being I chose the option early in the game to tell the Nilfgaardian that Sile blew up when Letho messed with her crystal in TW2. But I got the glossary update anyway as though I'd indicated she survived TW2.
Not exactly major or game breaking, but it does sort of hurt immersion when a character that was supposed to be dead in TW2 sort of shows up (albeit just in the glossary entry; she isn't actually in the cell with Rita).
