First of all, sorry for my poor english.
There is a few flaws that I really don't understand, first of all gates, there are six of them, but three don't have doors, or something to protect city (Gate of the Hierarch, Oxenfurt gate and Southern gate), it's just stupid, especially in middle of war, where city can be encircled and attack... just look at map http://mmo4ever.com/witcher/maps/novigrad/ how easy is attack this gates bypassing the fortified ones (to captured docks, attackers must go by one bridge without any fortifications, pass Hierarch gate, to bypassing two other, fortificated gates).
Next thing is this bridge with gate https://images2.alphacoders.com/714/714138.jpg turned in Novigrad side, it's also have not much sense, if this building is part of city defense, cause if army attack from the Sevens Cat Inn side http://mmo4ever.com/witcher/maps/novigrad-gustfields/ defenders on top of this gate, have no battlements to protect them...
Next big problem is, that is easy to bypass Redanians fortifications, becouse there are plenty of poorly protected bridges, without any fortifications around Novigrad.

It's look like, last Novigrad defense is Oxenfurt and this small Redanian camp on Pontar (http://mmo4ever.com/witcher/maps/velen-grayrocks/)... after Nilfgaard capture it, city will fall quickly, if Radovids army don't build more fortifications...
 
Map design in a game is not always realistic, there are trade-offs related to gameplay, technical limitations, or just not having enough time to get these kinds of details right (as most players will not care about them, they might not be high on the list of development priorities).
