I am having the same issues. Identical PC set up. I had to remove dsound.dll from the bin folder just to get it to play. Everytime I try to move with Geralt his body disappears.
Is 'dsound'dll' from a mod? I don't have it now as I always uninstall mods before a new patch comes out, and I did have it when I was using the weight change mod.

1. It will not allow me to install Ciri Alt DLC. The game is patched to 1.07 but it is not the latest base for the game.
2. NVidia GeForce experience doesn't seem to recognize the game settings in order to optimize.
3. Geralt disappears whenever I move. Like other than his head becomes a ghost.
4. Roach is still an idiot popping through walls and mountain sides to come when called.
5. Lastly, My FPS has dropped, not stabilized or increased.

I have a war horse of a machine and this is crap.
Below are some practices I always use to increase the potential PC games won't give me issues.

Very clean system. All I install from Nvidia is the driver you have to (first one in list, can't de-select it), and PhysX. Never Gameworks or anything else (Miracast, 3D stuff, HD Audio Driver). I don't need Nvidia setting my settings or bunging up my system. I use NVCPL for that.
I recommend;
-#1 - Remove all mods completely and cleanly
-Completely uninstall the Nvidia drivers
-Install Nvidia driver and choose 'custom'
(I'm using 353.30 that's not official but a hotfix for something)

-When installing new only install the main driver and PhysX. (if you use the HDMI cable for sound you might need to install the HD Audio Driver. I don't).
-IMPORTANT When installing the driver make sure you check 'Perform a clean installation' at end of the list.
-Don't run with MSI, Fraps, Mods or anything.
-Try the game.
-Then setup Nvidia Control Panel for SLI.
-Use the Witcher3.exe in NVCPL.
The recommendations above are a complete shot in the dark. Basically I'm just recommending starting with your system as clean as possible, get the game working, then add overhead back.
(not a recommendation) By the way, my game is installed on a SSD and my Windows swap file is on a dedicated SSD partition, and I have forced its min and max sizes to be the same at 48 GB. Crazy? Unnecessary? Hasn't caused me problems and it helped with Batmobile stutter in Arkham Knight.
