Works for me on a modded game.

Dubya, if there are still monsters on the area, regardless of how close they are, Geralt will keep his sword out. If you kill all the monsters in an area, the game will auto sheathe. It's working correctly.

Can you remove change 19 rom the list? I got the wilhelm scream before when the day 1 patch was barely installed.

I wish that depth of field strength was almost always in effect. I love how it makes everything so much more cinematic and realistic looking; it's always blasting in my modded Skyrim games.

Thanks Brogan, except it is really not working for me at all! Not sure why, since I am not using any mods that would affect that.
But if this is not a general problem, then I guess I'm stuck with it.

I'm having issues with it as well. In one HoS cutscene, using headphones with Master Volume at 90, Voice at 100, Music at 20 and Sound Effects at 40 I could barely hear Geralt's lines.

Try going to known locations and finishing off the monsters you know to be in a general areas, like the dogs south of Hang Man's Tree. When the final dog is dead, the combat music stops and if you don't hit any buttons, Geralt will sheathe his sword.

At least that is how the vanilla game works. If you've no mods installed that mess with it, then yea, something is corrupted or deleted likely.
