After testing a theory, I've confirmed that it's probably not intentional.
A few minutes ago, I loaded a game with the nudity censor turned off. I removed V's shirt, then re-saved the game to create a point of reference, and returned to the main menu to turn the censor on.
When I loaded the game again, V was acceptably clad in her infamous purple undergarments, top and bottom. At this point, I saved the game again, exited to the menu again, and re-disabled the nudity censor.
When I re-loaded the saved game, V appeared completely naked in the inventory menu, but was still wearing the purple undergarments in the first-person view. No amount of fiddling in the inventory screen could remove them, nor was I successful when I returned to the main menu and again tried to toggle the censor on and off.
So, it's not intentional on the part of CDPR; the nudity censor toggle is just broken. It seems that the setting of the 'nudity' flag is baked into the saved game and disabling the censor doesn't turn it off again.
