Quen explodes multiple times when attacking enemies that are blocking

Doesn't happen only with shielded enemies, it will randomly explode like that against all kinds of enemies when you attack them, perhaps when they parry or block. Not sure. I'm playing patch 1.04 on PC and these are my equipped skills: http://prntscr.com/7asnn6
