Regarding the decision to scale back the nudity, if the game was left in that condition then no one would be able to stream it on any of the streaming platforms without facing sanctions. It would be hard to make a game in 2020 that is not compatible with content creators that want to make youtube content, pro gamers that want to stream, etc.
Regarding the ethics of refunding a video game.... in my entire life I have sought refunds twice. Both times I sought refunds were on the basis of the game failing to work as advertised in a material aspect.
What I mean by that is you don't refund a game because an NPC was floating in the air, visual glitches, etc. You only refund when the game fails to work as a reasonable person would agree it to be either "unusable" or the experience being so bad that it is simply not enjoyable.
I have a hard time thinking of scenarios where one was able to play the game to completion, yet the game did not work in a material way.
In my view, people who refund a game they were able to complete are the ones that should be blamed for the cases of others that do have an unusable game but cannot get a refund because the studio has been forced to create criteria that is almost impossible to get a refund when it is truly warranted.
I have even seen news articles talking about people who want to refund the game, but keep it. WTF?
