Focusing on Triss and Yenifer is more appropriate, let Gerald "decide" who of them is best for him. I want to see relationship not animal attraction, Gerald is too old for running after every "available skirt"..

It couldn't actually.

Soon as you actually have visible penetration, pretty sure the rating would go to X (or whatever your country has above Mature).

If you think there won't be something at least as sexually explicit as the Elven Baths sequence, go read the ESRB for The Witcher 2, it had exactly the same "Sex Summary". Expect all that mocap goodness, and unless you somehow thought there was going to be actual penetration, you definitely will not be disappointed.
