If you start the game with no mods whatsoever, does that change anything?

Tried verifying the game cache via GOG/Steam?

Maybe it's related to your graphic drivers, tried updating them?

If all else fails I guess it might be time for a reinstall of the game.

Sorry for asking again but that mod looks modular which is good but can you point out only the file to get the black steel sword scabbard and not the silver one. I am currently using this mod for swords http://www.nexusmods.com/witcher3/mods/885/? , i only need the steel black sword.

That is because the sword mod you are already using are modifying both swords, the author of that mod needs to split the silver and steel swords in to separate files.
