Sounds & Geralt comments from bottom to top, weird stuff.
When doing the new DLC Quest about the pigs.
Inside the cave, Geralt comments the traps (which there is none of).
He is actually commenting things that is found above the cave, in the world.. part of another quest.
Also portals below in caves can be heard clear as a whistle on the ground.
Seem to me sound effects and encounter system work like this in the world:
Geralt on top of a mountain.
