That's something that's needed but I doubt that comes under 'dialogue tweaks' as it would require a change in the game mechanics. For Triss, Act 1 is fine, the only change needed is better early game dialogue for Yen. A possible more complete fix to problems that exist even with the tweaks after finding Ciri would be moving Now or Never into act 2 (between the Uma quest and The Isle of Mists), and adding one more (optional) quest to the Novigrad quest line in act 1. However, that would have required more work. But it would also avoid the inconsistencies in Skellige and Kaer Morhen (Ugly Baby), and not force new players to make a choice too early before knowing Yennefer well enough.
I think act 3 is more or less OK after the tweaks, it is a relatively short and linear part of the story anyway that is focused on defeating the Wild Hunt (Bald Mountain, Final Preparations, Battle Preparations, and the final battle). Maybe something could have been added in the epilogue, where it is only possible to talk to Ciri (witcheress ending). Also after the battle of Kaer Morhen and Vesemir's funeral, but there is at least the "broken bed" tweak before the battle. To deal with this, the sorceress pretended that nothing had happened and the witcher, as is his wont, sheepishly masked his feelings with a mix of hesitant interest and fake indifference. It must be said that both of them handled the situation quite awkwardly
Personally i really like the way CDPR make Geralt pursue Triss in W3 Btw this codex part come from negative choice :shuriken:

If following the intended order of the quests (by the level numbers), then a travel from Novigrad to Kovir to Kaer Morhen is clearly shorter than Novigrad -> Skellige -> Velen (for Uma) -> Vizima -> Kaer Morhen (see world map). And Geralt normally does the quests for Yennefer, Cerys, and Hjalmar first on Skellige, so that takes some time. Although Keira should be in Kaer Morhen, too, if still alive, but there is at least a short explanation about that before the battle. OK, maybe the deal with Tankred takes a long time, but there could at least be an explanation given, similarly to Keira. That is because the dialogue remains identical to the Yennefer (or no one) choice most of the time, there are simply very few consequences.
