Quests
1. Redanias Most Wanted, "Speak with Triss/Yennefer about Phillipa's Crystal" The journal gives you a failed objective depending on which sorceress you talk to, unavoidable red x
2. Contract: The Oxenfurt Drunk "Kill the Vampire" quest gives you a red x failed objective the moment you fend off the vampire while geralt is drunk and beckons the vampire to attack him. There's no reason why this should be a failed objective, because there's No way to kill the vampire until after you chase him to the hut.
3. Contract: In the heart of the woods, quest gives you a failed objective depending on whose side you choose and the method of disposing of the Leshen, choose Siding with Sven and up to a certain point you won't be able to do the ritual, likewise if you choose the ritual, you will be unable to do Svens marked individual option, choosing a method of dispatch will give you a red x in the journal for failing to do the alternative option... Dumb punishment considering you succeed in disposing of said Leshen with one choice, instead we should be given a green tick for actually deciding and following through with a choice.
Besides these three unavoidable failed objectives, every other quest in the game it is possible to completely flawlessly succeed in every objective and get a green tick on every sub objective.
The three unavoidables above are unfixed quests and probably will remain that way as I am unsure whether fixing them would be possible without bugging something else.
I just wanted to post this out there for any person trying to accomplish a completionist save with a flawless journal, I have done around 400 quests and it is unfortunate that these quests there is no workaround, I tried various different methods such as proceeding with both choices in the heart of woods quest, attempting to complete both via step by step progression, and that quest fails you the moment you get so far with either choice, oxenfurt drunk is linear and there's no way to avoid the vampire I tried running away from it to the hut but you get a fail because you have to defend yourself you cannot run, you simply won't proceed in the quest, and it's silly that you cannot speak to yennefer And triss about the phillipa crystal, they obviously want you to choose one sorceress over the other as a love interest, still it shouldn't give you a red x, in all honesty Geralt merely discussing the crystal with both sorceresses would be harmless and doable.
I completed Count reuvens treasure, Get Junior, Gangs of Novigrad, all green ticks no red x's and those are probably some of the most difficult quests to flawlessly blitz.
Hope this information helps completionist come to terms with the fact that you cannot perfect this game. I truly tried my best and encountered these three bugged quests.
